Sustain Grant Interest Form

To download the Sustain Grant documents and access the Temelio (grant application) platform, please fill out this interest form. Oregon Well-being Trust collects the information on this form so we can follow up with you in the future.

When you submit the form, you will be taken to a new page with links to the Sustain Grant documents. Download these documents to your computer for reference. You will also get a link to the Temelio platform.
